Information Collection, Use and Access

We collect and use the information provided on this form in order to provide advice, information and assistance in response to your concern. We are authorized to collect information for this purpose by section 11549.5 of the California Government Code.

You do not have to provide any personal information, and we recommend that you only provide the personal information that you think we need to respond to your concern. If, however, you do not give us enough information, we may not be able to contact you or help you resolve your concern.

It is the policy of the Office of Privacy Protection to share your personal information only as strictly necessary to respond to your concern. This may include disclosing your personal information to a business you complained about and to other government agencies. We also could be required to disclose the personal information that you provide in the following circumstances:

  • In response to a Public Records Act request, as allowed by the Information Practices Act;
  • To another government agency as required by state or federal law;
  • In response to a court or administrative order, a subpoena, or a search warrant.
